Your Mission

As a Care Path Designer you will help shape the future of healthcare delivery. You'll be
- creating new models of chronic care together with Medical Experts and patients, and then bringing them to life on our platform. Your work will directly influence how thousands of patients experience care every day.

Your scope of responsibilities

How we work:

• we work with Medical Experts, Providers and Pharma partners to identify chronic care pathways that could be transformed with our asynchronous care platform, connecting care teams with chronic patients beyond visits

• we identify & prioritize the needs of specific users - doctors, nurses or coordinators as well as patients

• we design and implement new, improved care pathways, leveraging our platform

• we measure impact on clinical outcomes, provider economics, as well as patient and staff experience, and do our best to continuously

• we use Figma, Slack, Typeform, Maze, Miro, Notion for daily communication and work

Our requirements

• Proven experience in service design, user research, preferably in the healthcare sector
• Strong ability to identify, analyze, and prioritize the needs of Healthcare Professionals (HCPs), Patients, and Providers.
• Demonstrated experience in mapping and designing customer / patient journeys and care pathways, and translating insights into structured, scalable processes.
• Hands-on skills in rapid prototyping and quick testing of new service concepts (e. G. through
  - ups, pilots, MVPs).
• Experience in facilitating workshops and
  - creation sessions, and leading multidisciplinary teams through discovery and design processes.
• Comfortable with driving organizational change – from mapping journeys to implementing new ways of working in
  - world healthcare settings.
• Ability to guide multidisciplinary teams through discovery, ideation, and implementation phases.
• Strong collaboration skills – ability to
  - create solutions with medical experts, product teams, and operational stakeholders.
• Working with B2B partners, ie. through agency or consulting experience
• Excellent communication and stakeholder management skills
• Data-driven mindset – ability to define and monitor metrics to evaluate and optimize care pathways.
• Curiosity and adaptability – comfortable working in a
  - paced, innovative healthtech environment.
• Experience in the medical or healthcare space (clinical practice, healthcare consulting, digital health, pharma, or provider systems) is a great asset.
• Fluency in English (additional European languages are a plus).
